Home & Community Care Officer - Carmen Denniss
Aims:
- To develop and maintain local community service provider links and networks. To be aware of and refer to other agencies when needs arise.
- To Network with health workers to provide education and awareness of health issues.
- To inform Aboriginal Community members of and to assist their access to health screenings, clinics and other health programs.
Covering the areas of:
- City of Wodonga
- Shire of Alpine
- Shire of Indigo
- Shire of Towong

- Are you a frail older person?
- Do you have a disability?
- Do you have a person that cares for you and you think they could use assistance?
- Are you a carer of a frail older person with a disability?
- Do you feel you need some support in caring for your loved ones?
- Would you like some information regarding what services may be available?
- Do you need to take a break?
